The term “hardship” is a government term where they make exceptions to the rules. Keep in mind, that just because you are in a hardship doesn’t necessarily mean that a withdrawal of this type is your only option. This following is not a form to be used for normal distribution requests. A hardship distribution, if permitted by the plan, must be due to an "immediate and heavy financial need". It also requires you to meet all criteria put forth by the Internal Revenue Service and your companies 401k Plan Document. Please note, the approval of this request is also pending the authorization of your Plan Trustee.
